In order to check if there is a problem in our theme, we suggest that you temporarily activate one of the default WordPress themes and try adding the Form.
If in this case, the form is working properly, please give me your WP Admin login credentials in the private reply so I can check what’s going on, directly on your site.
PS: I just checked the Plugin in our Local Host test environment and it works perfectly fine.
So, in your case, if the above steps didn’t help you, try to see if there is no conflict with your active plugins.
To check that, deactivate ALL active plugins except the HubSpot and try to add the form. If the form works now, activate the plugins one by one until the problem reappears, on that way you can find the problematic plugin(s) if any…
